All Affordable Care Act (ACA) 1095-B forms have been processed and mailed to all applicable participants that had medical coverage during 2020. Participants may also view current or prior forms and print additional copies by logging into the participant portal and visiting the myHealth section. In addition, the participant portal offers access to a myriad of valuable information regarding your health benefits, such as PPO providers/facility search, benefit information, various downloadable forms and many tools and resources to view claim history, deductible and out-of-pocket status, request ID cards and more to keep your health data at your fingertips.
