What does
Livongo offer?

Whole health care tackles many of the issues, that combined, can wreak havoc on one’s overall health. Christian Brothers Health Benefit Services is offering additional programs, at no cost to you, through Livongo, to help you take charge of your health.

You may already be familiar with the programs offered through our partner Livongo to help with diabetes and hypertension. All participants in the Employee Benefit or Religious Medical Trust, have free access to Livongo by Teladoc Health, now offering support in the areas of diabetes, hypertension, diabetes prevention and weight management.

Livongo is a program created to empower all people with chronic conditions, including, diabetes, and high blood pressure, to live healthier lives and reduce risk for serious health issues.

Diabetes Prevention and Weight Management

Living a healthier life

133 million Americans are living with diabetes or prediabetes, but 84% of them don't know they have it yet. The Livongo Healthy Living and Diabetes Prevention program is available through our Teladoc Health benefit and can help you if you are at risk for type 2 diabetes. The program doesn’t cost anything and helps you focus on living a healthier life. It also helps you lose weight and provides you with a smart scale to automatically track your progress.

Within the program, you will have access to a CDC-recognized program that focuses on lifestyle behavior changes to achieve health goals through:

Effortless data collection: Cellular scale provides seamless weigh-ins and food and activity tracking to understand lifestyle habits.
Personalized health signals: Lessons provide evidence-based strategies for healthy living and health challenges to drive small changes for big wins!
Human-centered approach: Coach-led meet ups for support and accountability and 1:1 live coaching from Livongo expert coaches.

Depending on your health goals, you could also receive a blood
pressure monitor and/or blood glucose monitor.

Diabetes Management and Support

Personalized health programs to help manage diabetes

Livongo for Diabetes is designed to support individuals
diagnosed with type 1 or type 2 diabetes and is
designed to make living with diabetes easier by
providing you with a connected meter, unlimited
strips and lancets and coaching at no additional
cost to you.

After enrolling, you will be shipped the
Livongo Welcome Kit that includes the Livongo
meter and all the strips and lancets needed to check
blood sugar. In addition, you will receive access to the
Livongo member website, My.Livongo.com, where your
program can be personalized and readings can be accessed.

Hypertension Management and Support

Monitor and
manage blood
pressure

The Livongo for Hypertension program helps
track your high blood pressure by providing
an automatic blood pressure monitor
connected to a smartphone app that
sends data to Livongo; an easy-to-use app
and dashboard; and guidance
on healthy habits.

Enrolling in the Livongo programs
allows you access to smart devices,
support when you need it, all
at no additional cost.
